Great article. These questions had always been something I've considered in games and movies. "How does the story react to the "side effects/collateral damage" of the protagonists.
On one hand, it is part of the media to let the audience ignore these aspects. But, on the other, it is such a rich area to be explored. I can think of many gameplay and narrative devices linked to this - such as actually raising awareness for "mass murdering" and encouraging a more stealth or pacifists nature; to even joke about it, such as in Rick & Morty's first episode.
